Frequently Asked Questions
How much will it cost me for my insurance coverage?
Medical and dental coverage for the employee is 100% paid for by TPSS Co-Op.
If you choose the Blue Preferred Medical Plan and/or the Traditional Dental Plan you will be
responsible for the difference in premium between the Blue Choice Plan and/or the Dental Network
Plan. **Money is deducted from your paycheck, if you are contributing to the cost of the premium, pre-
tax giving you a tax break. Please complete the Premium Only Plan form.
How do I enroll or make changes at open enrollment?
T o enroll in any of the coverages, you will need to complete the BenefitMall Employee election form
and return it to your HR Dept. If you need to make changes to your elections, terminate coverage for
you or your dependents then a BenefitMall Change Form is required and must be returned to your HR
Dept.
Can I enroll or make changes to my coverage outside of open enrollment?
There are certain qualifying events that would entitle you to enroll in the plan or make changes to
your existing coverage outside the open enrollment period. These include involuntary loss of other
coverage, birth/adoption, marriage/divorce, etc. These enrollments and changes must be made within
30 days of the qualifying event and will be effective as of the qualifying event date. In order to make a
change you must submit a BenefitMall Change Request form as well as supporting documentation.
Can I cancel coverage for myself or my dependents outside of open enrollment?
CareFirst coverages can only be canceled at open enrollment, or if your employment with TPSS Co-Op
ends or your hours of employment are reduced to under 30 hours per week. The Avesis coverage can
be canceled for the employee at any time. If an employee wishes to cancel coverage for dependents,
the dependents must have proof of other coverage. To cancel coverage, you must complete a
BenefitMall Change Request form. You may also be required to submit supporting documentation.
What happens to my coverage if I no longer work for my employer?
When your employment ends all of your coverages will terminate at the end of the month of your last
day of employment. COBRA will be available for those who qualify. COBRA is administered by TASC
and paperwork will be mailed out shortly after your last day of employment. For more information
