Eligibility




            Full Time employees working a minimum of 30 hours per week will be eligible after completing the new
            hire waiting period as indicated below:
              » Medical/Dental/Vision/HRA  - 1st of the month following 60 days of employment

              » Life and AD&D  -1st of the month following 60 days of employment

            Open Enrollment

            TPSS-Co-Op open enrollment period is May 1st - May 30th each year. This is the only time of the year
            when you can enroll, terminate, or make changes to your elections without a qualifying event. All
            changes, terminations and enrollment will be effective May 1st regardless of when the form is received
            in the month of May.

            If you are waiving coverage, you must still complete the enrollment form in your packet and check the
            waiver box for the coverage you are declining.





                        Contact your HR department for any changes, terminations, additions, or enrollments
                        outside of the Open Enrollment period within 30 days from qualifying event.
                        Supporting documentation will be required along with the appropriate enrollment/
                        change form.
