Life and AD&D

Life insurance will pay a cash benefit to your beneficiary in the event that
the insured passes. The Life and AD&D is offered through Mutual of Omaha.

» 1 x Salary to $250,000 maximum

» Policy payable to their named beneficiary

» NAPA pays 100% of the premium

Short Term Disability

Short-term disability (STD) is coverage that pays a percentage of an employee’s
salary when they are not able to work as a result of injury or illness. The Short Term
Disability is offered through Mutual of Omaha.

» 15 day elimination period for Accident and or Sickness

» Up to 11 weeks of maximum benefit
» 60% of salary to a maximum benefit not to exceed

$2,000 per week

» NAPA pays 100% of the premium

Long Term Disability

Long-term disability (LTD) is intended to protect your income for a long duration
after you have depleted your other options. The Long Term Disability is offered
through Mutual of Omaha.

» 66% of salary to a maximum benefit not to exceed

$12,000 per month
