Frequently Asked Questions




            How much will it cost me for my insurance coverage?
            Employees will pay $80 per month on a pre tax basis for medical coverage for employee only.
            Employees will also be responsible for 25% of the medical cost for their dependents; NAPA will pay
            75%. NAPA will pay 100% of the premium for Dental and Vision coverage for employees and
            dependents.


            How do I enroll or make changes at open enrollment?
            T o enroll in any of the coverages, you will need to complete the BenefitMall Employee Election
            form and return to your HR Dept.


            If you need to make changes to your elections, terminate coverage for you or your dependents
            then a BenefitMall Change Form is required and must be returned to your HR Dept.

            Can I enroll or make changes to my coverage outside of open enrollment?
            There must be a qualifying event that occurred within 30 days from the requested effective date.
            Qualifying events include but are not limited to involuntary loss of coverage, marriage, divorce,
            birth/adoption, etc.  You must complete either the BenefitMall election or change form and return
            it to your HR Dept along with proof of qualifying event within 30 days of the qualifying event.


            Can I cancel coverage for myself or my dependents outside of open enrollment?
            You can only cancel your coverage if you have a change in your employment status (employment
            terminated, reduction in hours, etc.) or become eligible and enroll in other employer sponsored
            coverage through your spouse etc.


            What happens to my coverage if I no longer work for my employer?
            Your coverage will terminate at the end of the month in which you are no longer employed.  You
            may be eligible to continue your benefits through COBRA after employment ends. NAPA uses TASC
            as their COBRA administrator. TASC will send your COBRA notice to your home. If you elect to
            continue your coverage through COBRA , then you will be responsible for the entire premium.
