Eligibility
Full Time employees working a minimum of 30 hours per week will be eligible after completing the new
hire waiting period as indicated below:
» Medical/Dental/Vision > First of the month following date of hire
» Life and AD&D, STD and LTD > First of the month following 30 days from date of hire
Open Enrollment
NAPA's open enrollment period is May 1st- May 31st each year. This is the only time of the year
when you can enroll, terminate, or make changes to your elections without a qualifying event. All
changes, terminations and enrollment will be effective June 1st, regardless of when the form is
received in the month of May.
If you are waiving coverage, you must still complete the BenefitMall employee election form in your
packet and check the waiver box for the coverage you are declining.
Contact your HR department for any changes, terminations, additions, or enrollments
outside of the Open Enrollment period within 30 days from qualifying event.
Supporting documentation will be required along with the appropriate enrollment/
change form.
