How much will it cost me for my insurance coverage?
Public Citizen's will pay 100% of the premium for the employee and their children. If the employee
chooses to cover their spouse they will be responsible for 100% of that premium.
How do I enroll or make changes at open enrollment?
T o enroll in any of the coverages, you will need to complete a BenefitMall Employee Enrollment form
and return to your HR Dept.
If you need to make changes to your elections, terminate coverage for you or your dependents then
a BenefitMall Change Form is required and must be returned to your HR Dept.
Can I enroll or make changes to my coverage outside of open enrollment?
There must be a qualifying event that occurred within 30 days from the requested effective date.
Qualifying events include but not limited too involuntary loss of coverage, marriage, divorce, birth/
adoption, etc. You must complete either a BenefitMall enrollment or change form and return it to
your HR Dept along with proof of qualifying event within 30 days from qualifying event.
Can I cancel coverage for myself or my dependents outside of open enrollment?
You can only cancel your coverage if you have a change in your employment status (employment
terminated, reduction in hours, etc.) or become eligible and enroll in other employer sponsored
coverage through your spouse etc.
What happens to my coverage if I no longer work for my employer?
Your coverage will terminate at the end of the month in which you are no longer employed. You may
be eligible to continue your benefits through COBRA after employment ends. TASC will mail to your
home address on file the COBRA packet with instructions on how to proceed.
