Eligibility

Full Time employees working a minimum of 30 hours per week will be eligible after completing the
new hire waiting period as indicated below. Part time employees that work between 17 1/2 and 29
hours in a consecutive work week for more than 6 months are also eligible for medical and dental
benefits. Long Term Disability is available to employees work 20 hours or more in a consecutive week.

» Medical/Dental - First of the month following date of hire
» Long Term Disability - First of the month following date of hire

Open Enrollment
Public Citizen's open enrollment period is July 1st - July 31st each year. This is the only time of the year
when you can enroll, terminate, or make changes to your elections without a qualifying event. All
changes, terminations and enrollment will be effective July 1st, regardless of when the form is received
in the month of July.

If you are waiving coverage, you must still complete the enrollment form in your packet and check the
waiver box for the coverage you are declining.

Contact your HR department for any changes, terminations, additions, or enrollments
outside of the Open Enrollment period within 30 days from qualifying event.
Supporting documentation will be required along with the appropriate enrollment/
change form.
